In my redesign of the Submit A Service Request Form, I reformatted the form design to have a vertical flow, created a more modern interface, and simplified interactions.

• Added a “same as previous” feature so the user doesn’t have to enter the same information multiple times

• Added a more modern interface for uploading a documents and photos

• Margins, padding, and gaps were made consistent between items

• Warning banner and informational paragraph were condensed to convey information in a more straightforward manner

• Instead of having descriptions next to input fields, fields are filled with example inputs before the user enters information

• Form is designed with a logical, vertical flow instead of being divided into two columns
